Background
Indonesia has the highest human mortality from highly pathogenic avian influenza (HPAI) A (H5N1) virus infection in the world.
MethodsA survey of households (N=2520) measured treatment sources and beliefs among symptomatic household members. A survey of physicians (N=554) in various types of health care facilities measured knowledge, assessment and testing behaviors, and perceived clinical capacity.
ResultsHouseholds reported confidence in health care system capacity but infrequently sought treatment for potential HPAI H5N1 signs/symptoms. More clinicians were confident in their knowledge of diagnosis and treatment than in the adequacy of related equipment and resources at their facilities. Physicians expressed awareness of the HPAI H5N1 suspect case definition, yet expressed only moderate knowledge in questioning symptomatic patients about exposures. Self-reported likelihood of testing for HPAI H5N1 virus was high after learning of certain exposures. Knowledge of antiviral treatment was moderate, but it was higher among clinicians in puskesmas. Physicians in private outpatient clinics, the most heavily used facilities, reported the lowest confidence in their diagnostic and treatment capabilities.
ConclusionsEducational campaigns can encourage recall of possible poultry exposure when patients are experiencing signs/symptoms and can raise awareness of the effectiveness of antivirals to drive people to seek health care. Clinicians may benefit from training regarding exposure assessment and referral procedures, particularly in private clinics. (Disaster Med Public Health Preparedness. 2016;10:838–847)

The multicomponent Dry Creek site, located in the Nenana Valley, central Alaska, is arguably one of the most important archaeological sites in Beringia. Original work in the 1970s identified two separate cultural layers, called Components 1 and 2, thought to date to the terminal Pleistocene and suggesting that the site was visited by Upper Paleolithic huntergatherers between about 13,000 and 12,000 calendar years before present (cal B.P.). The oldest of these became the typeassemblage for the Nenana complex. Recently, some have questioned the geoarchaeological integrity of the site's early deposits, suggesting that the separated cultural layers resulted from natural postdepositional disturbances. In 2011, we revisited Dry Creek to independently assess the site's age and formation. Here we present our findings and reaffirm original interpretations of clear separation of two terminal Pleistocene cultural occupations. For the first time, we report direct radiocarbon dates on cultural features associated with both occupation zones, one dating to 13,485-13,305 and the other to 11,060-10,590 cal B.P.

Summary
Introduction
Nutrition concerns the chemicals required by an organism for growth, tissue maintenance, reproduction and the energy necessary to maintain these functions. Many of these chemicals are ingested with the food, but others are synthesized by the insect itself. In some insects, microorganisms contribute to the insect's nutritional requirements. Achieving optimal nutrition involves a complex interplay between feeding behavior and post-ingestive processing of food. Insects must eat appropriate amounts of suitable foods, but avoid ingesting harmful excesses of toxins and nutrients. In this chapter we begin by considering the nature of nutritional requirements (Section 4.1). In Section 4.2 we set out the ways in which insects maintain nutritional balance, both by adjusting feeding behavior and post-ingestive processing. Next, we discuss the consequences for growth, development, reproduction and lifespan of failing to maintain nutrient balance (Section 4.3). Finally, we detail the nutritional contributions made by symbiotic microorganisms (Section 4.4).

Most insects have qualitatively similar nutritional requirements because their chemical compositions and metabolic capabilities are broadly uniform. Variation among insects arises from adaptations to particular diets or associations with microorganisms that provide specific nutrients.

Summary
Introduction
The lumen of the alimentary canal is a portion of the external environment over which the insect has great control, and this control is exerted predominantly by the gut epithelium. The principal functions of the alimentary canal are to process ingested food, mostly by chemical modification but also, in some insects, by mechanical disruption, and then to assimilate the products of digestion. The alimentary canal also comes into contact with toxins and microorganisms, including pathogens, associated with the food. Some of these microorganisms become resident in the gut and are beneficial to the insect. It therefore has to combine three capabilities: to mediate chemical transformations involving enzymes capable of degrading the insect's tissues; to provide an absorptive surface for the assimilation of nutrients and water; and to protect against pathogens and noxious compounds. These diverse capabilities are achieved through regional differentiation of the alimentary tract and integration of structure and function at both the molecular and whole-organ levels. Furthermore, the insect alimentary tracts include spectacular examples of adaptation in structural organization and function to different insect diets and habits.
This chapter is divided into four sections. Section 3.1 describes the structural organization of the insect alimentary tract. It is followed by Section 3.2 on digestion and Section 3.3 on absorption of nutrients, ions and water, and concludes with Section 3.4 on the gut as an immunological organ. Throughout the chapter the variation in structure and function with insect phylogeny and diet is addressed.

The cells of all animals, including insects, are bathed in an extracellular fluid (ECF); and most cells of the animal exchange solutes with the ECF, not the external environment. In most animals, including insects, solute exchange between cells and ECF is facilitated by the bulk flow of ECF, powered by one-to-many pumps (including hearts). The physiological system mediating bulk flow is known as the circulatory system, the topic of this chapter. Most insects have one major pump, the dorsal vessel, and multiple accessory pumps, with the circulating portion of the ECF known as the blood or hemolymph. The insect circulatory system is open, meaning that the hemolymph flows freely around insect organs, in contrast to the closed circulatory system of vertebrates in which blood is retained within vessels. The structural organization of the insect circulatory system and the determinants of blood flow are described in Sections 5.1 and 5.2, respectively, and the composition of hemolymph is considered in Section 5.3.
Hemolymph has four key functions. It is the vehicle for transport of hormones and nutrients between tissues, and a site for storage of some nutrients and water; these two functions are considered in Section 5.3. It is also a crucially important component of the insect immune system. The humoral (non-cellular) immune function is addressed in Section 5.3 and the cellular function in Section 5.4. Other insect organs also contribute to insect immunity, notably the gut (Chapter 3), fat body (Chapter 6) and Malpighian tubules (Chapter 18).

Recent Genome-Wide Association Studies (GWAS) have identified four low-penetrance ovarian cancer susceptibility loci. We hypothesized that further moderate- or low-penetrance variants exist among the subset of single-nucleotide polymorphisms (SNPs) not well tagged by the genotyping arrays used in the previous studies, which would account for some of the remaining risk. We therefore conducted a time- and cost-effective stage 1 GWAS on 342 invasive serous cases and 643 controls genotyped on pooled DNA using the high-density Illumina 1M-Duo array. We followed up 20 of the most significantly associated SNPs, which are not well tagged by the lower density arrays used by the published GWAS, and genotyping them on individual DNA. Most of the top 20 SNPs were clearly validated by individually genotyping the samples used in the pools. However, none of the 20 SNPs replicated when tested for association in a much larger stage 2 set of 4,651 cases and 6,966 controls from the Ovarian Cancer Association Consortium. Given that most of the top 20 SNPs from pooling were validated in the same samples by individual genotyping, the lack of replication is likely to be due to the relatively small sample size in our stage 1 GWAS rather than due to problems with the pooling approach. We conclude that there are unlikely to be any moderate or large effects on ovarian cancer risk untagged by less dense arrays. However, our study lacked power to make clear statements on the existence of hitherto untagged small-effect variants.
